Whole Life Farm Music School
Parent/Guardian Participation Agreement and Waiver
Whole Life Farm Music School is a program of Whole Life Farm Arts & Music Foundation. Please read the following agreement carefully. A parent or legal guardian must acknowledge and agree to these terms before the child may participate.
- Permission to Participate
I give permission for my child to participate in Whole Life Farm Music School programs. Activities may include music instruction, singing, playing instruments, creative movement, group games, performances, and supervised indoor or outdoor activities. - Acknowledgment and Assumption of Risk
I understand that participation may involve inherent risks, including slips or falls; physical movement; contact with instruments, cords, stands, sound equipment, furniture, or other participants; loud sounds; outdoor terrain; insects; weather conditions; and other risks ordinarily associated with music, movement, group, and outdoor activities. I acknowledge these risks and voluntarily allow my child to participate. I agree to inform program staff of any medical condition, allergy, disability, behavioral need, or other circumstance that could affect my child’s safe participation. - Emergency Medical Authorization
If my child becomes ill or injured and I cannot be reached, I authorize Whole Life Farm Music School personnel to contact emergency medical services and obtain reasonably necessary emergency treatment for my child. I understand that program personnel are not medical professionals and that I am responsible for medical or transportation expenses not covered by insurance. - Release of Liability
To the fullest extent permitted by Michigan law, I release and hold harmless Whole Life Farm Arts & Music Foundation and its directors, officers, employees, instructors, volunteers, and agents from claims arising solely from the inherent risks of my child’s participation. This release does not apply to gross negligence, reckless or intentional misconduct, or liability
that cannot legally be waived. - Behavior and Safety Expectations
I understand that participants must follow staff directions, treat others respectfully, and use instruments, equipment, facilities, outdoor areas, and animals, if applicable, only as directed. Staff may contact me and require early pickup if my child’s conduct presents a safety concern or substantially disrupts the program. Serious or repeated unsafe behavior may result in dismissal from the program. - Photo and Media Permission
I give Whole Life Farm Arts & Music Foundation permission to photograph or record my child during program activities and to use those images or recordings on its website, social media, newsletters, grant reports, and promotional materials. My child’s full name will not be published without separate permission.
